Second Passport (Duplicate)

You are applying for a second (duplicate) passport because:

-You have a valid U.S. passport

-you have frequent international travels which requires a first U.S. passport to be in an embassy for visa processing while you travel with a second passport

OR

-You have a visa in your passport (Israel, Iraq, etc.), which may prevent obtaining a visa for or traveling to certain countries


 

#1 – Fill out State Department Application online

Your first step will be to fill out your application online.  The U.S. Passport Agency will not accept hand written applications.  You will need to go to the State Department’s website (www.travel.state.gov ) to fill out your application, form DS-82.  Once you enter your information onto the application, you will click “Create Form” which prepares the application for printing.  The form has a 2-D barcode, which the State Department can scan into their database.  Please print two (2) copies of the application and be sure to sign both!  Please CLICK HERE for the DS-82 online application.

 

#2 – Ship Documents to Beltway Visa and Passport for service

Once you have printed and signed your DS-82 application you are ready to ship to us.  Please send us the following items:

 

1. Your completed and signed DS-82 applications

2. Your current passport

You need to submit your current valid U.S. Passport to obtain a second U.S. passport.  Your current passport will be returned to you along with your second passport.

3. Letter of Explanation

This letter should be addressed to the Department of State and should specify why you are applying for a second passport.  Please give as much detail as possible.  Please sign the letter and be sure to include your current U.S. passport number in the body of the letter.

4. Passport  Photos

You must submit two (2) identical passport sized (2” x 2”) photos.  Please do not attempt to take your own passport photos.  They can be rejected for many reasons.  Most pharmacies, photo studios or FedEx offices can take quality passport photos for you.  Photo requirements are:

-white background

-full face, front view

-photos must be identical and taken in the last six months

-Do not wear a head covering unless for religious reasons (please provide explanation)

-wear normal daily attire (no uniforms, costumes, etc.)

-if you normally wear a wig or glasses you may wear them for the passport photo

5. Proof of departures

You must submit copies of your travel itineraries listing all of your upcoming international travel.  Copies of airline tickets are also acceptable.  You should provide proof that you are traveling internationally for consecutive dates that will not allow normal visa processing procedures.

6. Beltway Visa and Passport – Passport Order Form and Payment

This is a one-page form that you should include with your documents.  It is your opportunity to let us know exactly what type of service you need, provide travel dates, special instructions, shipment information and payment information.  The U.S. Passport Office will charge $170 for a second (duplicate) passport.  Please CLICK HERE for our Order Form.  Please note that our service fee for our agent to process your passport request is in addition to fees collected by the U.S. Passport Agency.  Beltway Visa and Passport expediting fees are: $150 per passport for 5-day service and $200 per passport for next day service.

7. Beltway Visa and Passport – Passport Authorization Form

This form gives Beltway Visa and Passport permission to submit on your behalf at the U.S. Passport Agency in Washington, D.C. for expedited service.  It must be original signature in blue ink!  Please CLICK HERE to fill out the passport authorization form.

Please ship to us by a means that is traceable.  We suggest FedEx, UPS, or Express mail.  It is not advisable to send your passport or vital records through regular service via the U.S. Postal Service.
